Enrollment Application Part 2: Fee and Document Upload

Thank you for your interest in St. Barnabas School.

In addition to the St. Barnabas School online enrollment application, there is also a $150 application fee and required documents. If you are submitting an application for multiple children within your household, please complete a separate application fee and document upload for each child.

REQUIRED FOR ALL: birth certificate and baptismal certificate (if baptized into the Catholic faith), and Student Support/IEP/504 (if applicable).

REQUIRED FOR GRADES 2 - 8: most recent report card and standardized test results.

Documents required for upload must be submitted as legible scanned documents submitted as a PDF. Phone photos of documents are NOT permitted. If you prefer to submit your documents as a hard copy, please complete the demographic and payment components of this form, then stop by the Main Office (Monday-Friday 7:30am - 3pm), and we can make copies for you. Your application is not complete until all documents and the application fee are received.
