Required Medical Forms

MEDICAL FORMS - forms for the upcoming school year are due Sunday, August 18, 2024.  Students who do not have proper medical documentation on file may be excluded from school. Forms may be turned in at Information Sunday or mailed to the school throughout the summer ATTN: Medical Forms.
MEDICATION - Students who require medication during the school day must submit a MEDICATION AUTHORIZATION
ALLERGY- Students with food allergies must submit an ALLERGY ACTION PLAN
ASTHMA - Students with asthma must submit an ASTHMA ACTION PLAN
SEIZURE - Students with seizures must submit a SEIZURE ACTION PLAN
SPORTS PHYSICALS - All students participating in the St. Barnabas Parish Athletic Program must have an annual SPORTS PHYSICAL or MEDICAL FORM on file that authorizes sports participation. Please submit a completed form to the school for each participant prior to participation in the first program activity (practice, game, or event) of the school year.
