Required Medical Forms
MEDICAL FORMS - the following forms are due at the start of the school year
- Preschool - Medical Form
- Kindergarten - Medical Form, Dental Form, Vision Form
- 2nd Grade - Dental Form
- 6th Grade - Medical Form, Dental Form
- Every transfer student in Grades 1-8 is required to turn in a Medical Form, Dental Form, and Vision Form
MEDICATION AUTHORIZATION - Students who require medication must submit a Medication Authorization Form.
ALLERGY ACTION PLAN - Students with food allergies must submit an Allergy Action Plan
SPORTS PHYSICALS - All students participating in the St. Barnabas Parish Athletic Program must have an annual Sports Physical or Medical Form on file that authorizes sports participation. Please submit a completed form to the school for each participant prior to participation in the first program activity (practice, game, or event) of the school year.
