Medical Forms
MEDICAL FORMS for the upcoming school year are due Sunday, August 16, 2026. Students who do not have proper medical documentation on file for the school year may be excluded from school. Forms may be turned in on August 16 at Information Sunday or mailed/hand delivered to the school throughout the summer ATTN: Medical Forms.
SPORTS PHYSICALS
All students participating in the St. Barnabas Parish Athletic Program must have an annual SPORTS PHYSICAL or MEDICAL FORM on file that authorizes sports participation. This includes all St. Barnabas Sports from Preschool - Grade 8. Please submit a completed form to the school for each participant prior to participation in the first program activity (practice, game, or event) of the school year. - PRESCHOOL - Medical
- KINDERGARTEN - Medical, Dental, Eye Exam
- GRADE 2 - Dental
- GRADE 6 - Medical, Dental
- TRANSFER STUDENTS - entering Grades 1-8 are required to turn in a Medical, Dental, Eye Exam
ADDITIONAL FORMS, required for some
- MEDICATION - Students who require medication during the school day must submit a Medication Authorization
- ALLERGY- Students with food allergies must submit an Allergy Action Plan
- ASTHMA - Students with asthma must submit an Asthma Action Plan
- SEIZURE - Students with a seizure disorder must submit a Seizure Action Plan
